Let’s look at where you are:

Start asking money already now.
It will make it much easier for you in the future.
If you just charge a small amount now, Then you adjust it upwards.

One thing to remember before I go into the call structure:

DO NOT coach them and solve their problem in the Breakthrough call.
That will NOT make them your client and you don’t get to change their life.
