The first space shuttle built was the Enterprise. The Enterprise never flew into space because it was built without engines or a functional heat shield.

The Challenger- On January 28, 1986 space shuttle Challenger breaks apart after 73 seconds of flight.

The Columbia- On February 1, 2003 space shuttle Columbia disintegrated over Texas and Louisiana as it re-entered the Earth's atmosphere, killing all 7 passengers.
