WHAT DO WE DO FOR DIABETES

ROUTINE CARE OF DIABETES FOR PATIENTS AT BLACKBUTT DOCTORS SURGERY

ROUTINE DIABETES CARE

  • A care plan once a year
  • Care plan review every 3 months
  • Diabetes annual cycle of care every year
  • The aim is good control, and minimisation of complications

WHAT IS IN THE CARE PLAN AND DACC?

  • care plan is a plan of care, for diabetes and any other issues
  • care plan for diabetes lists the routine diabetes care
  • the care plan is made once per year, and ideally reviewed 3 monthly

WHAT IS IN THE DIABETES CARE PLAN?

  • a foot check with podiatrist every year
  • an HBA1C every 3 months
  • annual fasting cholesterol, and liver and kidney tests
  • an eye check every 2 years with the opthalmologist
  • dietician review
  • exercise physiologist or physiotherapy review
  • weight , blood pressure, review of symnptoms, problems and compliance very 3 months
  • influenza and pneumococcal vaccination
  • show diabetes care plan template

WHAT IS THE DACC

  • diabetes annual cycle of care
  • the care plan is to plan to do the work, the annual cycle of care is to check off the work done
  • to complete at DACC the patient must have 2x in last year height , weight ,bmi, foot check and BP, once in last 2 years eye check, and once in last year discussion re diet exercise and self care of diabetes, medications reviewed by doctor, once in last year urine and blood kidney check, cholesterol check and smoking status check.

WHAT IS THE BENEFIT OF DOING CARE PLAN?

  • team approach to diabetes, patient not bearing burden alone
  • access to 4 bulk billed appts with doctor and 5 with nurse, and up to 5 with allied health, for diabetes care
  • organise everything - no need to call for referrals, blood tests, scripts etc.
  • poeple dont do what you expect, they do what you inspect..!
  • trouble shooting problems that are preventing poor control before a whole year goes by

WHAT IS THE POINT OF THE DACC?

  • a bulk billed appointment purely to talk about diabetes
  • really a chance to see what was done, what are the results, and fine tune if targets are being missed
  • patient gets a print out of results to see their progress
  • show DACC print out

WHAT IS MEANT BY TARGET?

  • 30 mins of moderate physical activity on most if not all days of week
  • no smoking
  • lose 5-10% of body weight if overweight or obese
  • HBA1C of less than 7
  • sugars of 6-8 when fasting or 8-10 after a meal
  • total cholesterol of less than 4, HLD more than 1, triglycerides and LDL of less than 2
  • BP 130/80 or less
  • these goals dont suit everyone - every person is different and the doctor and patient have to work out together the individual goal, but we know people with diabetes live longer and healthier if we reach these goals

REMEMBER

  • good diabets control gets 20 years of low risk of complications
  • we must protect eyes, feet,kidneys, heart to prevent blindness, amputation and gangrene, kidney failure and death by heart attack or stroke.
