Preoperative Care

Preoperative Nursing Roles

  • Identify risks
  • Cultural/religious factors
  • Establish a baseline
  • Teaching
Baseline data includes lab/studies, medications, mental state, etc.

Risks - physiologic/psychologic.
Anxiety/fears
Look at health history - do a ROS
Allergies - meds, food, environmental
Medications - insulin, anticoagulants/antiplatelets, herbs/supplements, alcohol/recreational drugs

Determine need for interpreter, refusal of blood products, etc.

Teaching: Coughing/deep-breathing, amublation, devices, pain management, surgery specific info
NPO, need for prep

Patient Preparation

  • Identification/preparation
  • Chart completion
  • Patient belongings
  • Preoperative medications
  • Special considerations
Identification bands - ID, allergy, fall, restricted extremity, blood, refusal of blood products

Surgical site ID - indelible marker by surgeon, must be visible after prep/drape

Make sure all paperwork available - consents, anesthesia questionnaire, diagnostics, labs

Gown on, belongings and jewelry/prosthetics removed, sometimes site prep, application of TEDs/SCDs, warming

Common pre-op meds: antibiotics (ancef), anticholinergics (Trans-derm scop), Benzodiazepines (Versed), B-blockers (labetalol), H2 blockers (Zantac), opiods (Fentanyl)

Teaching: Coughing/deep-breathing, ambulation, devices, pain management, surgery specific info

Special considerations: sensory deficits, obesity, patient requests

Informed Consent

  • Adequate disclosure
  • Clear understanding
  • Voluntary
  • Nurses witness/advocate
Informed consent
-complete info on procedure, risks and benefits, alternatives
-opportunity for questions
-can be revoked at any time
-exceptions for minors, unconscious, mentally incompetent, life/limb situation
