Think about YOUR body:

when you think immobile- You as the nurse should think about DVT-- lovenox anyone?

Think Why

congenital Disease vs. Degenerative Disease
Congenital- Born With
Degenerative- acquired (can still be genetic) but also may be environmental triggers.

* hip dysplasia: http://www.choa.org/Patients-Families/New-Parents/Swaddling

Remember! A NEURO assessment is a big part of a musculoskeletal assessment. Neurologic dysfunction is a big part of musculoskeletal dysfunction.

review:
1) spina bifidia or myelomeningocele and its presentation. You need to review what it is, how it's treated and WHAT THE NURSES ROLE is in caring for these children.
2) SMA-or spinal muscular atrophy. review it's presentation and why these little frog legged kiddos really can't move a muscle.
3) Muscular Dystrophy
4) DMD- sub type- think about nursing care, prolonged bed rest and fracture.

Congenital Disease

what are some that we've talked about?
https://www.youtube.com/watch?v=6Ii_v3t9hpU

^^^^^ spina bifida- remind those mommies to take their folic acid! * remember with myelo kids you want to preserve the highest level of function you can. Parents are tempted to "do for them" but that is not the right thing to do. A baby may fuss for example about having to work hard to take a bottle and want to rely on tube feedings. However- if the child CAN bottle feed they should in order to preserve that function. If they can not-it's important that they suck on a pacifier when getting tube feedings so they can eat by mouth when they are stronger.

Think about what causes congenital disease: genetics, environmental factors in the womb, teratogens

^^^ what is cerebral palsy "CP kid"

Degenerative

think about the effects on their body
* muscular dystrophy
*mitochondrial disease

"healing broken battery disease" (DEBATE!)
SMA-different types
Diabetes -can be degenerative
LUPUS can be degenerative (autoimmune)
Marfan Syndrome

Infection

how can that cause immobility?
Botulism, Tetanus, Polio,
MENINGITIS!!! Think ABOUT the infection DESTROYING the nervous system and then resulting in permanent musculoskeletal impairment and immobility.

Gullian-barre-syndrome- very scary!
http://www.webmd.com/brain/tc/guillain-barre-syndrome-topic-overview

viral infection, bacterial infection- anything that destroys the nervous systems or musculature can cause PERMANENT immobility.
https://www.youtube.com/watch?v=zr2KUoRB-70--. very informative

^^^^^ a look at what POLIO used to do....

also a wonderful look at how an infectious agent can really cause impaired mobility.

injury

no one thinks it will happen until it does...

Sports Injury

the nurse's role in assessment...

ICE
well really RICE (rest)...

Ice to arrest blood flow + heat to encourage blood flow. If it's swollen it's not automatically ice.

If someone is in a sling or a cast remind them to keep it ELEVATED when sleeping or at rest. Blood return can be compromised and we don't want any swelling or damage to the tissue.

Contusion

More than a bruise...explosion into the soft Tissue
EXPLOSION OF PERIPHERAL BLOOD FLOW....
http://www.mayoclinic.org/first-aid/first-aid-bruise/basics/art-20056663

^^^^ remember Leukemia Nursing student. Every Bruise should have a story and every story should MATCH its bruise. Always be mindful that abuse is a possibility with ANY patient of any AGE, size, or socioeconomic status.

these injuries can damage multiple systems in the body at one time

Fracture

hairline and up...
http://learnpediatrics.com/files/2012/04/fractures.pdf

^^^^^ very helpful! You may want to make flash cards for the different kinds of fracture. Green stick fracture, vs Simple fracture etc. You will have questions on your exam that require you to know the different KINDS of fracture so familiarize yourself with those.

Fracture

what does it do to the body?
Fracture can mirror infection. Remember the contents INSIDE of your body that leak out of the place are extremely TOXIC to your own cells and your body will see it as a pathogen. Think about things that could be MASKING the body's cry for help such as sedation.

Epiphyseal Injury

can permanently affect growth

compartment syndrome from the ORTHO people.
(they take it pretty seriously)

Check PULSES, CAP REFILL, SWELLING, SENSATIONS!

NEURO And MUSC assessment go together!!!

YOU may have to cut one of these off someone so learn the DO's and DON'TS!!!

Spica Cast

all the way up the trunk
We talked a lot about this. Big reminder...see the nice bar there- NEVER GRAB IT!

^^^^ teach parents that as well (never trust grandma!)

Also know about HIp Dysplasia and why we use SPICA cast....

Let's talk about traction

and also counter traction...
http://www.healthline.com/health/traction#Overview1

^^^^ different types, why and your ROLE.

Let me see your Halo..

Cervical Traction
HALO traction. Remember skin integrity, infection can occur at the pin sites. Very vulnerable with a foreign body attached to the skeleton.

we are talking about

Types

  • Talipes varus: inversion or bending inward
  • Talipes valgus: eversion or bending out
  • Talipes equinus plantar flexion with toes lower than the heel
  • Talipes calcaneus: dorsiflexion with toes higher than the heel
  • I don't know why we kept them latin, but he thinks they're great.
okay- we got this! Talipes = foot

osteomyELitis

no using your limb

until the infection is healed silly.
