Chief Complaint
Heaviness on chest cavity
Migraine - due to lack of oxygen to the brain and lungs
Shortness of breath
Sharp,pains in upper chest kind of like acid reflux

History Of Present Illness
Hospitalized for pneumonia/ mucus blockage
Fever : 103.8
Dry cough - no phlegm but put on Zithromax Antibiotic, 4 liters of oxygen for 2 days then 2 liters with saline fluid to not dry out. IV for 4 days with solumedrol - steroids
Lortab 325 mg every 6 - 8 hours
Saline drip continuos

Past Medical History
Has been in and out of hospitals 13 times in her life for pneumonia, history of bronchitis, and mucous block
Allergies & Medications
Food allergies : watermelon, corn, strawberries, kiwi, dust, cats and tomatoes
Medication allergies: Benadryl, aspirin, and morphine
Medications: Theophylline 100mg every 12 hours
Ventolin inhaler as needed
Duo neb breathing treatments every 4 - 6 hours as needed through if flare ups
Pratropium bromide mixed with albuterol solution
Advair2 times a day
Multivimin and iron tablets

Divorced, in relationship
Alcohol - once a month
Tobacco - 4 - 5 times a day
Caffeine - 4 times a day
Not sexually active now
