In the Tell-Tale Heart the author lead us to believe that the man is not insane. We believe this statement is true because first of all in the story it states "The disease had sharpened my senses, not dulled them. We believe that his disease would be a hypersensitivity not a disability.

We think that the narrator knows what his consequences would be. The narrator said "I had no reason to kill him I loved the old man. Also the narrator said I soon found myself wishing they would leave." He got really agitated by the police. They were burning a hole inside his brain.
Our denial of him being insane would be because of his systematic actions. Examples would be when the author wrote"Oh I moved slowly,slowly so that the old man did not notice me." In this Sentence it can tell that he perfectly planned his plan. As a sane murderer would do he lied to the police and got guilty.
I think the narrator is in control of his actions because he made the plan so carefully that if he made one mistake he would get caught. some evidence would be "I could hear everything in heaven and in hell." Also he said "On the eight night I could feel it.It was the night.

plotting, planning and subterfuge proved he knew he was committing a crime.Some evidence would be "Now look how I went about it - with caution."Evidence-"Every night at midnight I unlatched his door and once I made an opening wide enough:I moved in slowly."This is a very carefully planned plan for a man with hypersensitivity.
Does the narrator know the difference from reality and fantasy? Our side would like to say that yes the narrator does know that he is not in a fantasy. We can support that answer from the text saying, "You wont when I tell you the wise precautions I took to conceal the body."He actually concealed the body carefully. But then his 'disease" came and he got so nervous that he exposed his plan to the police that were there.
