common Lesions

  • wounds
  • birthmarks/hereditary factors
  • allergies & psych (anxiety)
  • disease related (measles)
  • Acne
  • Ringworm (school aged kids)

review

  • Macule- Latin for "spot"
  • Papule- pimple like- red, enflamed no pus.
  • Vesicle- small fluid filled cyst or sac.

Wounds

  • Acute- should heal in 2 to 3 weeks
  • Chronic- Do not heal, associated with complications.

Infection

signs of Wound Infections:
increased redness ( erythema), edema, purulent exudate, pain, increased temperature, foul odor.

When does the nurse change the dressing?
As ordered and as NEEDED.

Beware of antipyretics that can "mask a fever" when treating for pain.

  • Rubeola, rubella, Varicella
  • Herpes Simplex I & II

It's Urushiol

poison IVY
Lesions are caused by urushiol from plants leaves and stems.

Sensitivity may develop after one or two exposures and may change over time.

Urushiol penetrates through the epidermis and bonds with the dermal layer.

Bites and Stings

most are harmless

Pediculosis Capitis

Head Lice
- common with school aged children

- Adult Louse lives only 48 hours without human host- female louse has potential life span of 30 days.

-Female lice lay eggs( nits at the base of hair shaft)

- Nits HATCH every 7-10 days

- Lice is really a war against the NITS

( stuffed animals in bags for 14 days, wash all the bedding, vacuum every day, wash all the clothing, get all new hair brushes and combs, wash and sterilize all towels, lice comb every day 2X per day for 14 days...)

Rickettsiae

Rocky Mountain Spotted Fever
Rash 2-4 days AFTER the FEVER
Bacterial- deadly if not treated.

Cat Scratch disease

Most Common Cause of regional lymphadenitis

follows the scratch or bite of an animal.

99% of the time it's caused by a cat or a kitten.

It is a benign self limiting disease.

it will resolve in 2-4 months- treatments including antibiotics are supportive.

Thermal Injury

Burns:

Toddlers: hot-water scalds
older kids- fire burns (candles, fires) kids playing with matches or lighters 10% of all house fires.

cigarette burns- child abuse


first degree- superficial - red no blisters.
second degree- partial thickness- blisters and the wound blanches with pressure
third degree- full thickness- Deep RED, WHITE, BLACK or BROWN- DRY SURFACE, thrombosed vessels visible, no blanching
fourth degree: full thickness and underlying tissue.
charring, insensate, amputation likely, sometimes can do a skin graft.

Emergency Care Priority: stop burning process, cover to protect the damaged tissue, get child to a burn center.

Debridement- Dressings.
