PURPOSE

  • Oral hygiene reinforcement
  • Periodontal treatment
  • Plaque control
  • Ultrasonic debridement

VARIABLES

INDEPENDENT VARIABLE

- Oral hygiene reinforcement during 4 following visits after a 1 hour appointment with debridement and OHI

DEPENDENT VARIABLE

- Plaque and Bleeding

TYPE & STUDY OF DESIGN

DESIGN

  • Parallel design
  • Test group
  • Control group

TEST GROUP

  • 1 hr ultrasonic debridement
  • OHI reinforcement at 4 appts
  • 3 month recall
  • OHI at weeks 2,4,8,9

CONTROL GROUP

  • 1hr ultrasonic debridement
  • OHI at 1st visit
  • 3 month recall

SAMPLING STRATEGY

STUDY

  • 6 month
  • Perspective
  • Controlled
  • Parallel
  • Randomized

TARGET GROUP

  • 44 people
  • Systemically healthy
  • Chronic periodontitis
  • Bone loss

SUBJECT EXCLUSIONS

  • Less than 18 teeth
  • Any systemic diseases
  • SRP finished within the last 12 months

METHODS

METHODS

  • BOP
  • PI
  • PPD
  • CAL

TYPES OF CALIBRATION

EXAMINER

  • 1 examiner
  • Calibrated on 3 subjects
  • 0.08-0.24 mm for PP
  • -0.26-0.28 for CAL
  • Blind study

INSTRUMENT CALIBRATION

  • Hu-Friedy XP-23/QW
  • 6 sites per tooth
  • Parallel to the long axis

OUTCOMES MEASURED

3 MONTH RESULTS

  • Reduction in bacteria
  • Reduction in plaque
  • Reduction in bleeding
  • ONLY in TEST GROUP
  • Disappeared at 6 mo.

POST 3 MONTH

  • Test group had PPD reduction

Untitled Slide

RESULTS

  • No significant difference
  • Significant improvement

CONTROL GROUP

  • Improvements in PPD
  • Lower PI & BOP
  • Reduction in PI and BOP

TEST GROUP

  • All around improvement
  • Greater PPD Reduction

COMBINED RESULTS

  • No difference between
  • - 5-7 mm pockets

CLINICAL SIGNIFICANCE

SIGNIFICANCE

Continuous OHI can help to form habits that that may be detrimental to oral health

AUTHORS INTERPRETATIONS

AUTHORS INTERPRETATIONS
- Lack of OH reinforcement. resulted in higher plaque and bleeding scores and accumulation of bacteria

AUTHORS INTERPRETATIONS CONT.
-Professional removal of dental biofilm every 3 months is beneficial in subjects with compromised plaque control

TREATMENTS

INCONSISTENCIES

  • Baseline measured
  • Same measurments taken at 3 & 6 mo.
  • Both had 1 hr debridement
  • Both had 3 month interval
  • Teeth polished

BIAS

  • Calibration on 1 examiner

VALIDITY

  • OHI was consistent
  • 1 Examiner screened and checked

OUR INTERPRETATIONS

OUR THOUGHTS

  • OHI is important
  • Do not want to overwhelm patients
  • Excessive OHI May become disregarded
  • Increased PI In test group

QUESTIONS ??

QUESTION 1
What experimental research group was used in this study?
A. Laboratory clinical trial
B. Cohort study
C. Randomized controlled
D. Animal research

Answer 1
C. Randomized controlled clinical trial- this specific type of scientific research tests a hypothesis and is used to test efficacy of specific interventions within a population where 1 group receives an Intervention (test group) and the other (control) does not

Question 2. What type of reliability is used when one examiner is calibrated
A. Inter-examiner
B. Intra-examiner
C. Co-examiner
D. Uni-examiner

Answer 2
B. Intra examiner- this is the type of calibration used with 1 examiner

Question 3 Which of the following is measured in the study
A. PI & BOP
B. BOP & PPD
C. CAL & PI
D. None of the above
E. All of the above

ANSWER 3

  • All of the above

Question 4 What was the intervention used in this study?
A. OHI at 4 consecutive appts
B. Full mouth debridement
C. Delivery of antimicrobials
D. Use of antiseptic mouth rinse

Answer 4
A. Oral hygiene reinforcement in 4 consecutive appts- this study of clinical and microbiological outcome of a 1 hour ultrasonic debridement of CPP with and without sessions of OHI reinforcement.

Question 5. What interventions were used on both groups to keep consistency
A. Screening with an x ray
B. 1 hour ultrasonic debridement
C. Use of local anesthesia
D. All of the above

Answer 5
A&B- these were all used as interventions to keep the clinical trial consistent in both groups

Question 6. What criteria excluded a patient from the study
A. Less than 18 teeth
B. History of systemic disease
C. Chronic perio
D. A&B
E. None of the above

ANSWER 6

  • A & B

Question 7 How many consecutive OHI reinforcement appts. Did the test group receive
A. 2
B. 4
C. 6
D. 8

Answer 7
B- 4 besides the initial and the 3 month OHI the test group received the 4 additional OHI appts

Question 8 Which of the following study methods reduces the bias in an experimental or evidence based study
A. Random sampling
B. Convienence Sampling
C. Judgmental sampling
D. Systematic sampling

Answer 8
A. Random sampling - this sampling allows everyone and anyone to have an opportunity to be included
