Steps for a successful transfer

• Patient identified as needing to be transferred to a higher level of care.
• Charge RN is contacted via vocera and/or MS Teams for assistance.
• 911 is called via vocera
• Documents pertaining to patient ( demographics, med list, history, EKG) are printed by clerk
• SBAR is pre primed by RN.
