Classification of Schizophrenia

Diagnosing Scizophrenia

DSM-V - Criterion A

  • a) Delusions
  • b) Hallucinations
  • c) Disoganised speech
  • d) Grossly disorganised or catatonic
  • e) Negative Symptoms
  • Two or more of Criterion A unless hallucinations are bizarre or voice is continuous or two voices

Criterion B - Social Occupational Dysfunction

  • Significant part of the time
  • one or more areas of functioning - markedly below level prior to onset

Criterion C - Duration

  • Continuous signs of disturbance - 6m
  • Must include at least one month of Criterion A symptoms (or less if successfully treated)
  • During non active periods disturbance may be limited to negative symptoms or two or more symptoms from Criterion A in attenuated form (odd beliefs / unsual perceptual experiences

Positive Symptoms

excess or distortion of normal functions

Hallucinations

  • perceptual distortions
  • exaggeration of any sense modality
  • Auditory - hearing voices are the most common

Delusions

  • firmly held but false beliefs
  • paranoid delusions
  • delusions of grandeur

Disorganised Speech

  • reflects problems organising thoughts

Grossly disorganised or catatonic behaviour

  • unmotivated or unable to complete a task
  • reduced reaction to immediate environment
  • An abnormal condition variously characterized by stupor/innactivity, mania, and either rigidity or extreme flexibility of the limbs).

Disorganized behavior may range from simple problems sustaining goal-directed self-care behaviors such as personal hygiene to unpredictable and bizarre socially inappropriate outbursts. For example, people may not dress according to the weather, (i.e., they may wear a heavy coat in the middle of summer), they may wear odd or inappropriate makeup, they may shout at people for no apparent reason, or they may mutter to themselves continuously, etc. They may even strip off their clothing and run naked through the streets, while chewing on road kill. Most anything is possible.

Negative Symptoms

reduction or loss of normal functioning

Avolition

  • reduction in self-initiated involvement in interests and desires

Affective Flattening

  • reduction in the range an intensity of emotional expression

Anhedonia

  • loss of interest in physical or social interaction
  • lack of reactivity to normally pleasurable stimuli

Alogia

  • Lack of speech fluency and productivity
  • Due to blocked thoughts
  • Short empty replies
