PRESENTATION OUTLINE
Infertility treatments at $100,000 per lifetime. See SPD for details.
Routine prenatal office visits covered at 100%. High risk or complications covered as regular office visit.
Preventive care is covered at 100% in all the plans and not subject to a deductible.
Vaccinations are covered as preventive and we've added coverage for travel vaccines.
We have a great orthodontia benefit. Our plan pays 50% up to $2,500 per individual per lifetime.
Wisdom teeth removal is covered under our medical insurance, not dental.
Injury to the teeth as a result of falling of a bike is covered under medical insurance, not dental.
Injury to the eye as a result of being hit by a baseball is covered under medical insurance, not vision.
You can put up to $5,000 in a dependent care account pre-tax to pay for childcare expenses for kids under 13.
Getting ready to send your child to college? I posted a health related check list for you.
And, if that child decides to backpack through Europe, there is a post on international medical insurance.
If you choose the HSA, you must meet the family deductible before any expenses are paid out.
You can cover children until they're 26, but only use money in your HSA as long as they are tax dependents.
If you don't have an HSA, you can put up to $2,650 away pre-tax for medical expenses for your family in an FSA. (Even if you don't have ASHA's health insurance.)
