Long Term Care Ombudsman Program
(Catholic Charities of Santa Clara County)

Mission Statement: "Catholic Charities of Santa Clara County serves and advocates for families and individuals in need, especially those living in poverty. Rooted in gospel values, we work to create a more just and compassionate community in which people of all cultures and beliefs can participate".

Description of services

Ombudsmen are people who advocate for elder living in nursing and residential care homes. It speaks up for frail, elderly, chronically ill, isolated, disabled, or otherwise at-risk long term care residents. This program supports the rights and well-being of elders in long term care facilities in Santa Clara County. Also, this program gives elders an opportunity for their complaints to be listened to and to be solved. In addition, this is a multilingual program which helps a broader group of elders when their first language is not English (English, Spanish, Mandarin, Japanese, Cantonese and others). Some of the services offered by this program are 1) complaint analysis and support, solve elder and dependent adult reports of abuse in nursing homes and residential facilities, witnessing of Advance Health Care Directives for elder in nursing homes and provide information regarding long term care services.

John XXIII Multiservice Center (by Catholic Charities of Santa Clara County)

This program offers a variety of services for elders. Some of the main services offered at the San Jose facility are an Alzheimer's program for Asian seniors and families, a health clinic administered by nurses and health insurance guidance. In addition, the staff is multilingual and helps elders who speak other languages such as Spanish, Vietnamese, Cantonese and Mandarin.

The Interim HealthCare brand encompasses home care, hospice and health care staffing services. Home Care is a diverse and dynamic industry. Services in the home may be necessary due to acute illness, long term health conditions, permanent disabilities, terminal illness or the impact of aging. Home health care personnel include nurses, therapists and other specialized professionals who provide clinical intervention such as wound care, assistance with medication, improved ability to walk, or teaching individuals and their families' self-management of their condition while remaining at home. Personal care and support services are non-medical and include companionship, homemaking and assistance with activities of daily living such as grooming and bathing. These services are usually provided by personal care assistants and home care aides as well as companions…

"The Medicare hospice benefit, enacted by Congress in 1982, is the predominate source of payment for hospice care. More than 88% (2012 NHPCO Facts & Figures) of patient days are covered by Medicare. Other payment sources include Managed Care or Private Insurance, Medicaid (same reimbursement as Medicare), self-pay (private pay), and uncompensated or charity care. There must be a life expectance of six months or less (if the disease runs normal course) and there must be physician certification of limited life expectancy. "
